Currently, the following items are required for all corresponding authors and will be displayed publicly (ie, in any applicable versions of your article, including preprints):

• Name
• Affiliation
• Address
• Phone number
• Email address

The following items will also be displayed publicly if provided, but they are not required:

• Fax number
